Most content about AI in healthcare falls into one of two failure modes.
The first is breathless optimism — press releases dressed as analysis, every announcement of a revolution, every demo a cure. The second is reflexive skepticism dressed up as rigor — endless caveats, regulatory handwringing, and a studied reluctance to acknowledge what's working in the field right now.
Neither camp is useful to the people doing the real work. Not to the physician trying to understand what AI can actually do in their practice before signing a contract. Not to the health system CIO weighing a build vs. buy decision on a $2M platform. Not to the engineer trying to understand why their RAG pipeline breaks down on messy EHR data. And not to the operator trying to explain to a board why the ROI on their AI investment is harder to measure than the vendor said it would be.
We've been building clinical AI at ThetaRho long enough to know the difference between what the demos show and what production looks like. We’ve had conversations that don’t make it into case studies. And we’ve developed some strong opinions about where this space is going — and where it keeps getting it wrong.
